When you've decided to apply for CHIP brought to you by KidzPartners, you have several choices. You can

- Call us anytime at 1-888-888-1211 or 215-967-4540 or email us at tmoore@healthpart.com (be sure to include your name and address in the email) to request an application be mailed to you. TTY users please call 1-877-454-8477 or 215-849-1579.
- Call 1-800-986-5437 to apply by phone. Representatives are available Monday-Friday, except holidays, from 7:00 am to 7:00 pm, and Saturday from 9:00 am to 3:00 pm to take your application over the phone.
- Apply online with COMPASS, Pennsylvania's online application system.
Only one application needs to be completed for all children in your household. To avoid delays, be sure to answer all questions, and include required current proof of income. Proof of income can be faxed to 215-967-9281 or mailed to:
